Company information missing from financial statements created by Statement Writer

A cover letter in QuickBooks Statement Writer (QSW) does not display the company name or dates in the fields where this information should display.
Instead of automatically populating the company name, the name may be left blank or display as <>.  The fields where dates are expected to show display as [compiled/reviewed] and [enter statement dates here].
How to fix it
Manually enter the company name into the cover letter or other document using the following steps:
  1. When adding a document to the statement (E.G. Compilation Letter, Cover Letter...), open the Document. 
  2. Within that document, replace the dynamic <> field with the written-out name of the company.
  3. If other fields are missing or failing to populate, such as the [compiled/reviewed] and [enter statement dates here] fields, then replace them as well.
  4. Save your changes to the document. 
  5. The manually-typed information will appear on your document when it is included in your statement.
